What makes a ring look vintage?

Three details do most of the work: milgrain, the row of tiny beads along an edge; engraving or filigree on the shank and gallery; and a slightly raised, ornamented setting rather than a plain modern head. Remove those and the same stone reads contemporary.

Is vintage detailing harder to maintain?

It collects more residue, because milgrain and filigree have more surfaces. A soft brush and warm soapy water every week or two keeps it bright. The detailing itself is durable; it is cast into the metal rather than applied.
